What do i put for employment status ?

I deliver bread for a bakery. I am a independent contractor. I own a distributing llc, but they pay me commission each week just like a paycheck. well on apps or whatever , when it ask me my employer , do I put my llc name or the bakery that pays me ? and when they ask for gross, do I put my gross business money before all expenses or what i pay myself. thanks
